ceftazidim iv 2000mg 3dd

alleen monotherapie indien geen ander evident focus (bv. huid, luchtwegen, lijn etc)  
Medication:

meropenem iv 1000mg 3dd
Remarks:

bij cefalosporine resistente Gram-negatieve bacteriën in (inventarisatie) kweken
Medication:

vancomycine iv 15mg/kg 3dd dd

or

vancomycine iv loading dose 15mg/kg, 45mg/kg continuous per 24 hours
Remarks:

bijstarten bij verdenking huid- of lijninfectie
Medication:

metronidazol iv 500mg 3dd
Remarks:

bijstarten bij mucositis

niet nodig indien meropenem iv is gestart
Remarks:

Indien hemodynamisch instabiel of opgenomen op de IC zie Sepsis (neutropenie)

empirische antigist-therapie wordt aanbevolen in hoog risico setting (IC-opname, enterocolitis) in combinatie met een van de volgende: persisteren van gisten in surveillance kweken of als patiënt geen anti-gist profylaxe heeft gehad

toevoegen bij neutropenie indien na 48u onvoldoende/geen respons

micafungin in het AvL en anidulafungin in het Amstelland ZH

General comments

Deze adviezen zijn bedoeld voor patiënten met neutropenie als gevolg van chemotherapie.
Neutropenie wordt gedefinieerd als: aantal neutrofielen < 0.5 * 109/L.

Bij de patiënt die met sepsis op ICU is opgenomen wordt in geval van ceftazidim of meropenem continue dosering aanbevolen, waarbij een oplaaddosis gegeven dient te worden.
