General comments
In het AvL is gekozen voor micafungine i.p.v. caspofungine.
Dosering micafungine: lichaamsgewicht ≤ 40 kg: 2 mg/kg/dag, lichaamsgewicht > 40 kg: 100 mg/dag. Bij onvoldoende reactie, bv. indien de kweken positief blijven of de klinische toestand niet verbetert, mag de dosis worden verdubbeld.
